
Preoperative planning
A mandatory step prior to surgery is preoperative planning. Determine optimal offset and leg length by using templates. Check the need for custom implants and bone transplants. It is also prudent to formulate the surgical approach and determine the size and placement of the femoral neck osteotomy, intramedullary plug and stem centralizer.

Positioning of the patient
Patient position should always be checked by the surgeon. It is very important that the pelvis be fixed as rigidly as possible. Any displacement of the hip during manipulation leads to a high risk of incorrect placement of the acetabular component.
Incision
Minimal trauma to soft tissues is achieved by careful splitting of muscles and partial excision of capsular soft tissue.
Transversal ligament
Remove central osteophytes, being sure to save the transversal ligament in order to obtain optimal pressurization when the cup is cemented.
