Improved cement penetration and increased interface strength are achieved when bone lavage is used.The concept of micro-interlock is well established and appears to be a positive contribution to the quality of fixations. Micro-interlock is the concept of creating a sound cement bone construct.
The interface strength is not only affected by the degree of cement penetration but also by the quality of the cancellous framework. The micro-interlock is the mechanical interlock between bone and cement. It is important to have an optimal micro-interlock in order to achieve a durable interface. Doughy cement applied by hand cannot be induced to penetrate and form interlock in a uniform and predictable fashion.
Doughy cement is easier to use but fails to intrude and interlock with bone. Low viscosity cement has good intrusion characteristics but is difficult to contain in the acetabulum.
There are a number of essential prerequisites for successful micro-interlock.6
